Important | Accessing Your Form 1095-C

In accordance with IRS regulations regarding the Affordable Care Act (ACA), all eligible employees are entitled to receive a copy of Form 1095-C (Employer-Provided Health Insurance Offer and Coverage). This form contains important information about your health care coverage that may be needed when filing your federal income tax return. While you do not need to attach Form 1095-C to your tax return, you should keep it with your tax records for the year.

How to Retrieve Your Form

You may access your 1095-C through either of the following methods.

Digital Access Recommended
- Log in to your Employee Navigator account at any time to view, download, or print your form.
- Find the download link under the Quick Links column on your home page.
Request a Paper Copy
- If you prefer to receive a paper copy via mail or have difficulty accessing Employee Navigator, you may submit a formal request to Human Resources.
