Submitting a Report or Filing a Formal Complaint

Mid strongly encourages anyone who experiences or observes any acts of sexual harassment or retaliation to promptly report the incident(s) and seek assistance from the College. The College can only take corrective action when it becomes aware of the problem.

A Formal Complaint is the document filed and signed by the Complainant, or the Title IX Coordinator, alleging a policy violation by the Respondent and is different than a report. The Formal Complaint is the document that officially requests the College to open an investigation. A Formal Complaint may be filed and accepted via paper or electronic submission that contains the Complainant’s physical or digital signature or otherwise clearly distinguishes the Complainant as the sender. If the notice is submitted in a manner that does not meet this standard, the Title IX Coordinator will communicate with the Complainant to verify their intent to file a Formal Complaint and ensure it is filed properly.

Any student, staff member, or other member of the college community who experiences, observes, or becomes aware of any acts of sexual harassment or retaliation is encouraged to report it to the College’s Title IX Coordinator. Reports, complaints, inquires or concerns may be made in person, via phone, regular mail, or email at any time (including during non-business hours) to the Title IX Coordinator whose contact information is listed below.

Anonymous Reporting

Anonymous Reports may be submitted through any of the means listed above. Mid recognizes an individual’s wish to remain anonymous. However, a person that is considering submitting an anonymous report should be aware that the College’s ability to respond may be limited, and may impede the ability to offer and/or provide supportive measures. The act of submitting a report does not carry an obligation to initiate a Formal Complaint, as explained above and in the College’s Title IX Sexual Harassment Policy.
